O. Unsuitability

1. By Commandant’s Direction. Commander (CG PSC) shall direct the discharge of enlisted members for unsuitability except as provided inSection 2.0.5. of this Manual. A discharge for unsuitability in lieu of disciplinary action will not be issued unless Commander (CG PSC) determines the Service’s and the member’s interests will best be served by administrative discharge. See Section 2.00 of this Manual when recommending the discharge of a first-term performer for unsuitability.

2. Causes for Discharge for Unsuitability. The purpose of discharges for unsuitability is to free the Service of members considered unsuitable for further service because of:

  • (1) Inaptitude. Applies to members best described as unfit due to lack of general adaptability, want or readiness of skill, clumsiness, or inability to learn.
  • (2) Personality Disorders. As determined by medical authority, personality behavior disorders and disorders of intelligence listed in reference (d), Coast Guard Medical Manual, COMDTINST M6000.1 (series), Chapter 5.
  • (3) Apathy, defective attitudes, adjustment disorders as listed in reference (d), Coast Guard Medical Manual, COMDTINST M6000.1 (series), Chapter 5, inability to expend effort constructively, or other observable defect for which a separation designator code (SPD code) exists that renders a member unsuitable for further military service.
  • (4) Unsanitary Habits.
  • (5) Alcohol Abuse. See Article 2.B. of reference (h), Coast Guard Drug and Alcohol Abuse Program, COMDTINST M1000.10 (series), for guidelines on alcohol abuse cases.
  • (6) Financial Irresponsibility.

A. Definitions.
9. Conditions or Defects not Physical Disabilities. Certain conditions and defects may cause a member to be unfit for continued duty and yet not have physical disabilities within the meaning of the law, thereby subjecting the member to administrative separation. These conditions include, but are not limited to, alcoholism; allergy to uniform clothing; character disorders; enuresis; heat intolerance with disturbances of thermal regulation; inability to be fitted in uniform clothing; motion/travel sickness; obesity; primary mental deficiency; pseudofolliculitisbarbae of the face and/or neck; somnambulism; stuttering or stammering; systemic or marked allergic reactions following stings by red ants, bees, wasps or other stinging insects; unsanitary habits including repeated venereal disease infections. A full listing of personality and intelligence disorders is contained in chapter 5 of the Medical Manual, COMDTINST M6000.1 (series).

40. Physical Disability. Any manifest or latent physical impairment or impairments due to disease, injury, or aggravation by service of an existing condition, regardless of the degree, that separately makes or in combination make a member unfit for continued duty. The term physical disability includes mental disease, but not such inherent defects as behavior disorders, personality disorders, and primary mental deficiency.

Section B. Psychiatric Conditions (including personality disorders).

1. General.

a. Initial assessment. The following diagnostic categories conform to Diagnostic and Statistical Manual (DSM) IV-R and indicate the appropriate reference for disposition. In determining qualification for appointment, enlistment, and induction, or appropriate disposition (when the condition has been determined to be disqualifying for retention in accordance with paragraph 3-F-16 of this Manual), the diagnosis appears under DSM IV Axis I or Axis II. Conditions generally considered treatable and not grounds for immediate separation, mental health treatment may be authorized for members when medically necessary to relieve suffering and/or maintain fitness for unrestricted duty. The decision to provide treatment for mental health conditions will be based on a review of all factors, including the opinion of experts, probability of a successful outcome, and the presence of other physical or mental conditions. If a successful outcome (availability for worldwide assignment) is not realized within six months of the initiation of therapy, the patient's condition must be reassessed. If the reassessment indicates that the prognosis for a successful outcome is poor, the member shall be processed for discharge pursuant to Chapter 12 of the Personnel Manual, COMDTINST M1000.6 (series), or through the Physical Disability Evaluation System (PDES), COMDTINST M1850.2 (series).

b. PDES Determination. Examination for purposes of PDES determination shall include a mental health evaluation performed by a military or VA mental health care provider. A military or VA mental health care provider is a psychiatrist, a doctoral level clinical psychologist, or doctoral level clinical social worker with necessary and appropriate professional credentials who is privileged to conduct mental health evaluations for the DoD, VA or the Coast Guard.

2. Personality Disorders. These disorders are disqualifying for appointment, enlistment, and induction under Section 3-D of this Manual and shall be processed in accordance with Chapter 12-B-16, Personnel Manual, COMDTINST M1000.6 (series). ...
a. 301.00 Paranoid.

b. 301.20 Schizoid.

c. 301.22 Schizotypal.

d. 301.4 Obsessive compulsive.

e. 301.50 Histrionic.

f. 301.6 Dependent.

g. 301.7 Antisocial.

h. 301.81 Narcissistic.

i. 301.82 Avoidant.

j. 301.83 Borderline.

k. 301.9 Personality disorder NOS (includes Passive-aggressive).

l. Personality trait(s) considered unfitting per paragraph 3-F-16.c.

Comments: Paragraph 3-F-16.c refers to mood disorders, bipolar disorders or recurrent major depression.
